About Hart House Supported Living Facility
Hart House is set in a lovely residential area based close to Southend On Sea town centre and Seafront. At Hart House we aim to provide a therapeutic environment by promoting a rehabilitation programme to meet the individual needs of those who have a mental health diagnosis.
Our outcome is to establish a conducive atmosphere in which each service user’s mental, social and spiritual needs are recognised and respected at all times.
We believe that individuals have a right to live an independent and autonomous life within the community with the help of our qualified and experienced staff to meet individual goals and aspirations and exercise freedom of choice, and also to support, assist and empower service users to gain the skills required for them to live a more fulfilled and meaningful life in society.
Hart House is an open supported living unit and is staffed 24 hours per day. Staff have vast experience working with people with dual diagnosis and those with a forensic history.
Support is delivered by incorporating various aspects, including social and practical skills training and risk management. Support plans are recovery-based and devised with service users’ maximum input. Continuous assessments are undertaken and reviewed to evaluate the changing needs of the service users.
It is envisaged that the average rehabilitation programme lasts for two years (longer if needed), and any eventual aftercare can hopefully support individuals living within the community.
Our rehabilitation principle adopts a view to gradually move service users forward to they may be able to achieve confidence, improve coping skills, be better equipped and empowered to lead a more fulfilling and independent life.
Hart House will endeavour to promote a rehabilitation programme whereby all staff will actively participate and support service users to achieve individual goals.
